I can't claim complete knowledge or what is possible, but I think you are right - the JavaScript APIs and VBA are the only cross-platform options (for Mac and Windows desktop) that allow you to control Word itself. Since Word clearly has all the necessary hooks to allow automation (presumably via the Apple Events mechanism, otherwise AppleScript could not work with it, and also presumably COM, otherwise Excel & PowerPoint VBA wouldn't be able to automate Word), in theory you might be able to control Word from.NET without VSTO, but AFAIK Microsoft does not yet provide any equivalent of the Windows.NET Interop assemblies that would let you do so. Nor do I know whether you could write your own framework to use COM (depends on whether that interface is available, since COM is a Microsoft-only implementation on Mac, whether it is supported - I doubt it - and whether you can work out how to use it). AFAIK the AppleEvents mechanism should be available but I have no idea how much work would be involved in making that work.
